Caregivers by WholeCare Launch Care Management Services

Last month, Caregivers by WholeCare, a non-medical home health company that offers continuum of care services, announced the launch of a new range of services called Care Management for seniors and disabled persons. WholeCare Transitional Care Services assist clients to successfully transition from a facility back to their home safely and efficiently post-surgery. The company also launched WholeCare Medication Management Services program to ensure individuals are getting the best therapeutic benefit from their prescriptions and taking the proper dosage.

The Transitional Care Services program is designed to ensure individuals recuperating from illness or injury are set up for success at home for healing and remaining as independent as possible for as long as possible. The package includes a comprehensive safety assessment and report on the status of the home before the patient is released. WholeCare staff review discharge and home health instructions, confirm follow-up doctor's appointments, and communicate instructions with the family.

The Medication Management Services comprise medication set-up and reminders by a WholeCare nurse using appropriate equipment or technology to ensure that the patient receives the right medications and the right dose at the right time.
